Applicants that require a fingerprint-based criminal history record check for employment purposes and need the results mailed back to the applicant’s address must use form “C”. (A Criminal Hit will result in the disclosure of convictions and pending data only) Employment - response returned to applicant O.C.G.A. § 35-3-34 provides that private persons and businesses may obtain the Georgia criminal history record of another person by providing a signed consent of the person whose records are requested on a form prescribed by the center, which shall include such person's full name, address, social security number, and date of birth. SSN may delay the processing of this form and the criminal record check. In order to be licensed, work at, or be present at, a licensed facility/organization, the law requires that you complete a criminal background check. (Health and Safety Code sections 1522, 1568.09, 1569.17, 1596.871, and 1796.19).

Requests for criminal record searches must be executed by the submission of a signed and notarized “Criminal Record Name Search” form SP-167.
